INITIAL APPOINTMENT
Please arrive 30 minutes early and to the correct clinic location for your initial appointment with Dr. Panchal. That will allow enough time to get you checked in and to complete all of the necessary paperwork.
DO NOT forget to bring the following with you to your appointment:
- New patient packet (completed before your arrival)
- A photo ID
- Your insurance card
- Copies of your previous MRIs, x-rays, CT Scans and any other diagnostic reports you may have. (They can be either on CD or films)
Diagnostic reports and films are required for the most successful first visit to the clinic. Any form of previous consultation letters and electrodiagnostic (EMG) reports should also be brought if they are available.
REFERRALS & MANAGED CARE PLANS
Health plans have specific guidelines and regulations you must follow in order to be seen by a specialist. If authorization is required by your health insurance company from a primary care physician to be seen by the specialist then please provide us with the necessary information and documents to the initial appointment.
PLEASE, BE CONSIDERATE OF OTHERS
If you must cancel an appointment, please inform our clinic at (972) 806-1188, preferably at least 48 hours before your scheduled appointment. This will enable us to schedule another patient who may be in pain or are needed to be seen.
